What is a Doctors Note?

A medical certificate or doctor's certificate is a written statement from a physician or another medically qualified health care provider which attests to the result of a medical examination of a patient. It can serve as a "sick note" (documentation that an employee is unfit for work) or evidence of a health condition.

What needs to be included on a Doctors Note?

A Doctor's note should include the following:

• confirms the legitimacy of time missed by an employee or student
• certifies a doctor appointment with a health care provider
• certifies the date(s) upon which the doctor visit occurred
• assess the patient's health condition
• recommends the amount of sick time to be taken
